For an ambulance to the hospital of your choice*, call 24 hours a day, 7 days a week: (833) 259-CEMS (2367). *when medically appropriate

My Checklist Along My Health Care Journey

• I fully understand all of my discharge instructions.
• I understand my diagnosis and symptoms to look out for.
• I clearly understand the name, dose, side effects, and purpose of all my medications.
• I understand if I need to make changes to my diet and whether I should limit or increase my activity. 
• My caregiver and I understand what kind of care I will need when I leave and what changes I will need to make at home.
• If I need help with transportation arrangements, I will let my medical team know.
• I have a plan to follow up with my health care provider(s).
• I know who to call in case of an emergency.
